BSA Medical Form Parts A, B & C

Both Scouts and Adult Leaders must file this with T840 before going on any camping or hiking activity. You must
also attach a copy of the front & back of your medical insurance cards when you submit these forms. 

Part C is needed for Summer Camp and Winter Camp. It must be signed by a medical professional (e.g., a doctor).

Please download this form, then go through the PDF to find instructions on how to fill it out and complete the
required YPT course. Once you fill it out, sign it, initial it, and bring it to Nidhi Goyal at the next T840 meeting 
along with your YPT certificate.
